BLUE CROSS BLUE SHIELD PHI AUTHORIZATION -  January 1, 2016 new PHI forms are available for those retirees and dependents who have not signed one. The forms on file prior to January 1st will not need to be redone. The PHI forms are needed on file at BCBS in order for us and our alternates to call on your behalf about claims or even ordering a new card. One will be needed for your spouse, dependents or surviving spouses. Forms will be at the union hall, our office, and on the UAW Medical Trust website at www.uawtrust.org. Please contact our office so we can get one for you and your spouse to sign.
