 UAW BENEFITS OFFICE UPDATES AND REMINDERS FOR UNIT 1 RETIREES

FEDERAL TAX WITHHOLDING CHANGE ON PENSION CHECKS

If you have noticed a decrease in your pension benefit, it may be because more federal tax and/or state withholding have been adjusted. State Street Bank is now calculating the tax withholding for federal and state taxes instead of Benefit Connect, the new administrator for benefits. You can change the withholding amounts for the next pension payment for review and re-adjust accordingly. You can always contact your tax advisor for the accurate amount you need to withhold when deciding to change your withholding amounts. If you want to make withholdings changes, we will be happy to help you or you can call Benfit Connect at 1-888-409-3300 between the hours of 7 am to 3 pm.

TRADITIONAL CARE NETWORK BCBS

  • Monthly contributions still $17/Single $34/Family
  • Deductible for In-Network increase of $15 now $400/Single; increase of $25 now $675/Family
  • Deductible for Out-of-Network same $1,000/Single $1,700/Family
  • Coinsurance still 10% for In-Network and 30% for Out-of-Network
  • Total Out-of-Pocket Yearly Max for In-Network increase of $45 now $800/Single; increase of $80 now $1475/Family
  • Total Out-of-Pocket Yearly Max for Out-of-Network no change $3,000/Single $5,550/Family

NON-MEDICARE (PRE-65) CO-PAYS

  • Office Visits co-pay still $25 for 6 visits with Primary Care Physician
  • Office Visits with a Specialist is still not covered
  • Urgent Care including Retail Health Centers still $50 co-pay
  • Emergency Room still $125 co-pay (waived if admitted)

MEDICARE (POST-65) CO-PAYS

  • Office Visits co-pay covered by Medicare at 80% after Part B deductible is met; you pay the remaining 20%
  • Office Visit with a Specialist covered by Medicare at 80% after Part B deductible is met; you pay the remaining 20%
  • Urgent Care including Retail Health Centers still $50 co-pay
  • Emergency Room still $125 co-pay (waived if admitted)

PPO (Preferred Provider Organization) PLANS/HUMANA (Plan Closed for New En-rollment)

  • Monthly contributions still $17/Single $34/Family

  • Deductible for Out-of-Network same $1,000/Single; $1,700/Family

No changes in cost share to MA plans

  • No monthly contributions
  • Deductible for In-Network $245 per person
  • Deductible for Out-of-Network $490
  • Coinsurance still 10% for In-Network 30% Out-of-Network
  • Total Out-of-Pocket Yearly Max for In-Network $630 per person
  • Total Out-of-Pocket Yearly Max for Out-of-Network $1,395 per person

NOTE:Certain drug classes will have generic-only coverage. Retirees currently taking brand-name drugs in the affected drug classes should have received a letter in the month of October instructing you along with your doctor, how to switch to a covered generic medication.

SPECIALTY MEDICATION CHANGES

In order to keep specialty medications affordable and to manage care, certain Specialty Medications will not be covered under the medical benefit and will need to be obtained through the pharmacy benefit. Specialty medications will still be covered under the Trust. A letter will be mailed with instructions on how to obtain the specialty medication through the pharmacy benefit.

FLU, PNEUMONIA, SHINGLES AND TDAP (Whooping Cough, Tetanus, and Diphtheria) VACCINATIONS

You can now go to a participating Express Script network pharmacy and show your Express Scripts prescription ID card and receive your flu, pneumonia, shingles, or Tdap immunization for the pharmacy co-pay of $12 until December 31, 2016. On January 1, 2017, the co-pay will be $14. Express Scripts customer service number is 1-866-668-0274 available 24 hours a day, 7 days a week.

Express Scripts Medicare Members - As of January 1, 2014 you will need to give permission to the Express Script Pharmacy to fill any prescriptions ordered directly by your doctor.  The Centers for Medicare & Medicaid Services (CMS) requires Express Script to make this change for both new and renewed prescriptions sent directly to the Express Scripts Pharmacy.  Express Scripts Pharmacy will contact you through automated phone messages and/or by mail to obtain approval.  The prescription will be filled once your permission is received.  There are 2 ways to give your permission and request your medication.  Contact Express Scripts Medicare customer service at 1-866-662-0274 (24 hours/7 days a week) at prompt press #1 or log on to www.Express-Scripts.com.  If you have not set up your account you will need to register.  Have our prescription ID card handy and select the prescriptions you want to receive through home delivery.

 


DIABETIC SUPPLIES - As of March 1, 2014 US MED is no longer a participating provider.  If you purchase supplies after that date, you will be responsible for the charges. EDGEPARK (1-800-321-0591) and MEDTRONIC (1-800-646-4633) are in-network providers.

BLUE CROSS BLUE SHIELD PHI AUTHORIZATION -  January 1, 2016 new PHI forms are available for those retirees and dependents who have not signed one. The forms on file prior to January 1st will not need to be redone. The PHI forms are needed on file at BCBS in order for us and our alternates to call on your behalf about claims or even ordering a new card. One will be needed for your spouse, dependents or surviving spouses. Forms will be at the union hall, our office, and on the UAW Medical Trust website at www.uawtrust.org. Please contact our office so we can get one for you and your spouse to sign.

 

 


BENEFICIARIES - Please make sure you have updated your beneficiaries. You can call Benefit Connect at 1-888-409-3300 or contact our office.  You will need to provide the name of the person you are designating, their birthdate, social security number and current address. When changing your address with us, our system does not cross over to the hall. Please contact the hall with your changes also 815.544.2111.

 


OPTIONAL DEPENDENT GROUP LIFE PREMIUMS- The cost of the Optional Dependent Group Life insurance (ODGL) premium has now been split to reflect the cost of payment for a spouse and/or children. Benefit Express mailed letters informing you to elect which dependent coverage you are paying for. If you do nothing, you will be paying for both. This applies for Surviving Spouses that continued the ODGL coverage as well. If you have not done this or need clarity, please contact us.

United health care 

UNITED HEALTHCARE - (contact UAW Benefits Representative).

United Healthcare (UHC) - Currently we are experiencing a lot of problems with United Healthcare. The Trust is working diligently with UHC to solve these issues. If you are enrolled in UHC and experiencing any problems, please let us know right away. If you would like to opt out of UHC you need to write a letter and address it to United Healthcare, P.O. Box 29675, Hot Springs, Arizona 71903.

*NOTE: If you have a Medicare card - DO NOT SHOW IT TO THE PROVIDER.

Always make sure you write down the name, date, and time when you call customer and always print the page if you go to the website.

 

 


LEGAL SERVICES REINSTATING - There is currently a part-time staff on site and they are not taking new cases as of yet. Attorney Black and Hart have retired and new attorneys need to be assigned to our location. The location and services as far as we know will still be the same; wills, power of attorney, name changes, bankruptcy, uncontested divorce, etc. We will post on the UAW Local 1268 website and Facebook page, and our office voicemail page the opening date once details become available.

 

 


$1,000 CAR VOUCHER - During 2015 National Negotiations, the Company agreed to provide all current, as of September 16, 2016 FCA LLC, retirees a $1,000 car voucher good towards the purchase or lease of a new Chrysler, Jeep, Dodge or Ram vehicle subject to the provisions below:

  • You may elect to receive either one (1) $1,000 or two (2) $500 vouchers for the purchase or lease of a new Chrysler, Jeep, Dodge or Ram vehicle.
  • If you elect to receive two (2) $500 vouchers *They may be “stacked” only for purposes of the purchase or lease of a Chrysler Town & Country, Dodge Grand Caravan , Ram pick-up, Jeep Grand Cherokee or Jeep Wrangler or *They may be used, individually, for the purchase or lease of any other new Chrysler, Jeep, Dodge or Ram vehicle.
  • The vouchers will be valid for vehicles transacted during the 12-month period following the launch of the program.
  • The vouchers are not transferrable to dealers or dealer personnel.

A web based system is being developed to allow retirees to register and receive the voucher(s). It is anticipated the online system is to be up and running on May 1, 2016. We will post the web address on the UAW Local 1268 website and have the info on our voicemails. NOTE: These vouchers are considered taxable income and a tax document will be issued in 2017 to each retiree who elects to receive the voucher(s) regardless if it is used or not for purchase or the lease of the vehicle.

Mopar Parts - are available for retirees at a 25% discount good for one (1) year by calling 1-800-852-6672. You are allowed six (6) discounts per year. You will need to give your last name and your CID (Corporate Identification Number).

 

 

Retirees who are receiving paper checks will experience longer delays in receiving their pension checks. We encourage you to sign up for Direct Deposit of your pension check. Your UAW Benefit Representatives can assist you. We will need your bank’s name, routing number and checking or savings account number.
